DOS proposes changes to Form DS-1648 (Application for A, G, or NATO Visa)

    The Department of State (DOS) issued a notice and request for comments on proposed revisions to Form DS-1648 (Application for A, G, or NATO Visa). This form is used by foreign diplomats and government officials, representatives to international organizations like the U.N., and NATO representatives and staff.

    DOS is making the following changes to the form:

    • Providing additional instructions on the DS-1648 landing page.
    • Adding a question to the Passport page: “Do you currently hold, or have you previously held, nationality of another country other than specified above?” [Yes/No]
    • If yes, adding the following field and question: “Other Country/Region of Origin (Nationality)” and “Do you hold a passport for the other country/region of origin (nationality) indicated above?” [Yes/No] → If yes, provide the following information: “Passport Number” [text box].

    Comments are accepted until May 11, 2026.
