May 15, 2025The Department of State (DOS) issued a 60-day notice requesting public comment on proposed revisions to Form DS-71, Affidavit of Identifying Witness. The form is submitted in conjunction with a U.S. passport application when the applicant is unable to establish their identity to the satisfaction of the official reviewing the application.
Comments will be accepted for 60 days, until July 14, 2025.
