May 15, 2025The Department of State (DOS) issued a notice requesting public comment on proposed revisions to Form DS-71, Affidavit of Identifying Witness. The form is submitted in conjunction with a U.S. passport application when the applicant is unable to establish their identity to the satisfaction of the official reviewing the application.
The notice solicits comments on whether the information collection in DS-71 is necessary, how the quality, utility, and clarity of the information collected can be enhanced, and how to minimize the reporting burden on those who are to respond.
Comments will be accepted until July 14, 2025.
