April 16, 2020On April 16, 2020, ACF/ORR provided notice regarding its intent to seek expedited OMB review of an information collection request related to UAC facilities and care providers and provided a 60-day period for submission of public comments on the proposed information collection. In particular, ACF is requesting that OMB grant a 180-day approval for this request under procedures for expedited processing. The information collection consists of ten forms with the stated purpose of monitoring UAC facility compliance with federal laws and regulations, legal agreements, and ORR policies and procedures. Specifically, the components of this information request include the following forms:
- Care Provider Facility Tour Request (Form A–1A)
- Notice to UAC for Flores Visits (Forms A–4 & A–4s)
- Authorization for Release of Records (Form A–5)
- Program Level Event (PLE) Report (Form A–9)
- Emergency Significant Incident Report (SIR) and Addendum (Forms A– 10A & A–10B)
- Significant Incident Report (SIR) and Addendum (Forms A–10C & A– 10D)
- Sexual Abuse Significant Incident Report (SA/SIR) and Addendum (Forms A–10E & A–10F)
- UAC Satisfaction Survey (Forms A– 11 & A–11s)
- UAC Satisfaction Survey Aggregate Data
- Hotline Alert (A–12)

On October 29, 2020, ORR issued a notice proposing to continue the information collections originally approved for 180 days under emergency approval. ORR has removed two of the ten original forms included in the request. Specifically, ORR no longer plans to implement the UAC Satisfaction Survey (Form A–11 & A–11s) or the UAC Satisfaction Survey Aggregate Data instruments.

Revisions to approved information collection
On January 6, 2021, ORR invited public comment on revisions to the approved information collection. ORR plans to revise six of the eight approved instruments. Four of the revised instruments will be incorporated into ORR's new case management system, UAC Path. The other two will remain PDF instruments. In addition, ORR plans to add two new instruments that will be incorporated into UAC Path (Notification of Concern (Form A-7) and Event (Form A-9). Finally, ORR plans to remove one currently approved instrument from this collection (Hotline Alert (Form A-12)). ORR is providing the public with a 60 day comment period.

Under the Biden Administration, on September 27, 2021, ORR requested public comment within 30 days of publication regarding plans to revise six instruments, add four instruments, remove one instrument, and replace the term "unaccompanied alien child (UAC)" with "unaccompanied child (UC)."
Instruments for revision include Notice to UC for Flores Visits (Forms A-4 & A-4s), Authorization for Release of Records (Form A-5), Emergency Significant Incident Report (SIR) and Addendum (Form A-10A), Significant Incident Report (SIR) and Addendum (Form A-10B), Sexual Abuse Significant Incident Report (SA/SIR) and Addendum (Form A-10C), and Program-Level Event (PLE) Report and Addendum (Form A-10D). ORR plans to discontinue Hotline Alert (Form A-12).
