May 4, 2017Department of State (DOS) issues new supplemental questionnaire (Form DS-5535) for those "who have been determined to warrant additional scrutiny in connection with terrorism or other national security-related visa ineligibilities." The questionnaire asks applicants for their passport numbers, travel history, employment history and residential addresses over the past 15 years, the names of all spouses or partners (whether living or deceased), and applicants' user names on all social media accounts they have used in the last five years.
