    DOS requests comments regarding proposed revisions to Form DS-3077, Request for Entry into Children’s Passport Issuance Alert Program. This is a revision of a currently approved collection. Form DS-3077 is necessary in order for DOS to determine the admission into the Child Passport Issuance Alert Program, which allows parents and those with custody of children to find out whether their U.S. passport applications have been adjudicated.
